The SNAP IV Form is a screen for ADHD.  It is to be completed by parents and teachers.

The MCHAT is a developmental screen for children between 16 and 30 months. 

The SCARED questionnaire is designed to assess anxiety disorders in children 8 to 18 years old.

The Beck's Depression Inventory is a self-reported questionnaire to screen children 13 years and above for depression.

The Ages and Stages Questionnaire is a developmental screening tool that pinpoints developmental progress in children between the ages of one month to 5 ½ years. This form is for patients aged between 1 month to 2 months 30 days old.

CAST (Child Autism Spectrum Test)

The CAST form is a questionnaire to screen for autism in children ages 5-11 years old.

The WEISS rating scale is a questionnaire that evaluates the extent to which an individual's ability to function is impaired by any emotional or behavioural problems. This form is to be complete by the patient

This WEISS form is to be completed by a parent or guardian.

The GAD-7 screen is a questionnaire to screen for anxiety in children ages 12 and older.
